Hello All,

I am new so not sure if best forum for this, but just wanted to ask within this group.

Currently overseeing development of a new program at a community hospital and actively involved in decreasing the use of high risk  medications (i.e. Promethazine, Donnatal, Codeine for cough, etc.) along with medication dosage errors (most of which are caused by use of standard adult dosages on pediatric patients). We have collected lots of information and presented some of it at the International Quality and Safety Forum in Amsterdam this past Spring.

Question is if anyone else doing similar work and if so is there a pool of information of how significant these problems are nationwide? Especially in community hospitals, which are mainly geared to adult care. Also, some of the different strategies and techniques that have been used with positive, or negative, results.

To say the least the experience has been a shock to me coming from and working for a major academic center. My own personal naiveness, I guess. :)
